چکیده

Introduction The terms ‘inferior’ and ‘recurrent’ laryngeal nerve have been used interchangeably to describe a branch of the thoracic vagus that loops around the subclavian artery (on the right) or aortic arch (on the left), and then ascends to terminally arborize [1]. The ILN carries sensory, motor and parasympathetic fibers, and divides into an internal branch (sensory to the vocal cords and subglottis) and external branch (motor to the intrinsic muscles of the larynx except cricothyroid). In as many as 70% of cases, this branching is extralaryngeal, predisposing a branch of the nerve to injury. In the vast majority of these cases, this bifurcation occurs more than 1.0 cm from the cricoid cartilage [2, 3]. ILN dysfunction is among the most common, feared and litigious complications of cervical explorations, and is associated with temporary or permanent vocal cord dysfunction. When bilateral injury occurs, the morbidity is even more dramatic, often requiring tracheostomy.

Pseudotumors and mimickers of malignancy of the head and neck pathology.

We are summarizing some of the difficult pitfalls in tumors of the head and neck, which we have encountered in our biopsies referred for consultation as well as from our routine praxis in the last 20 years. Shortly we are presenting the following lesions of head and neck: multifocal sclerosing thyroiditis, mucoepidermoid carcinoma of the thyroid, solid cell nests, Chievitz organ, rhomboid gloss...

Adenomas of cervical maldescended parathyroid glands: pearls and pitfalls.

BACKGROUND Missed parathyroid adenoma (PTA) is the commonest cause of persistent hyperparathyroidism. Although many are subsequently found in well-described locations, some are found in unusual regions of the neck. This paper presents the combined experience of three large tertiary endocrine surgery centres with maldescended PTA (MD-PTA). METHODS Patients were recruited from the endocrine sur...

How to Avoid Injury of the External Branch of Superior Laryngeal Nerve

Pearls and Pitfalls in Head and Neck Surgery Introduction The EBSLN is the main motor supply to the CTM. The contraction of this muscle stretches the vocal fold, especially during the production of high frequency sounds [1] . Therefore, EBSLN paralysis leads to an important impairment of voice performance, mainly among women and voice professionals. This nerve crosses the superior thyroid vesse...

Intraoperative photodynamic detection of normal parathyroid glands using 5-aminolevulinic acid.

OBJECTIVE It is important to identify and save the normal parathyroid glands during head and neck surgery because of their role in regulating the blood calcium level, yet it is often difficult to localize normal parathyroid glands during surgery. Fluorescence-guided parathyroidectomy in patients with hyperparathyroidism has already proved useful. However, there are few reports of fluorescence-g...

Structure-Inherent Targeting of NIR Fluorophores for Parathyroid and Thyroid Gland Imaging

The typical method for creating targeted contrast agents requires covalent conjugation of separate targeting and fluorophore domains. In this study, we demonstrate that it is possible create tissuespecific near-infrared fluorophores using the inherent chemical structure. Thus, a single compact molecule performs both targeting and imaging. We use this strategy to solve a major problem in head/ne...
